ID Sticker kit contains:
Two waterproof ID cards - One to use now, and one to use when you need to update the information.
Each ID card includes spaces for name, physical description, contact phone numbers, medical info, consent to treat (for emergency situations) and more.
One highly reflective ID Sticker "sleeve" - Just slide the completed ID card into this sleeve, and remove the cover from the "sticky back". Your ID Sticker is ready to adhere to a smooth surface, like the side of a bicycle helmet.
Its bright yellow color makes it noticeable, and the words, "ID INSIDE" and "IN CASE OF EMERGENCY PULL TAG" are plainly visible.
One printed decal - Again in bright, reflective yellow, this self stick decal can be placed on the opposite side of a helmet from the ID sleeve.
The words, "IN CASE OF EMERGENCY" AND "ID ON HELMET" appear on this decal. However, the words "ON HELMET" can be easily cut off to use the decal on a wheelchair or elsewhere.
