Custom Allergy Labels

• This kit will provide enough custom allergy labels to ensure that your child's lunch box, cup, locker, back pack, etc. are all marked with clearly printed, easily readable allergy information, putting your mind at ease.

Custom allergy labels can be hard to find, but can be priceless when your child has severe, life threatening allergies! Lucas is lucky, he's only allergic to pollen (the photo above was just an exmple). But we know students who are allergic to everything from Latex to wheat.

These custom allergy labels are personalized with your child's name, and up to four allergies on them. The bright yellow color ensures that they're seen, and they're waterproof, so they can go on cups, lunchboxes, back packs, car seats, lockers, etc.

You will receive a form to fill out with your child's name, and 4 blanks for listing allergies. Please fill this form out carefully, as your order will be printed exactly as you have filled out the form.

Just mail the form back in the postage paid return envelope, and you will receive 60 printed custom allergy labels with your child's name and the allergies you requested listed.
